I don't think that is satisfactory. If you have the model on a photoshoot session why not ask for the rest;
Year of birth, Height, Current location, Location of birth, Contact info tel, Contact info email.
Create a standard form for this, almost all other sites have that.

This should appear in the main forum as it's not a technical issue but a suggestion.
These models move around so much that it's impossible to keep regular tabs on them - if other sites are providing contact information then I doubt much of it actually works.
As far as physical appearances ... well you've 100+ photos and videos, the camera never lies ...
