Right now there are only two rows of information about a certain model:
Date of Birth and Location.
I don't think that is satisfactory. If you have the model on a photoshoot session why not ask for the rest;
Year of birth, Height, Current location, Location of birth, Contact info tel, Contact info email.
Create a standard form for this, almost all other sites have that.
